A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF PLAINVIEW
APPROVING THE RENEWAL OF THE LOCAL HEALTH DEP~ CONTRAC~
(TDH DOCUMENT NO. C5000552) BETWEEN THE CITY OF PLkINVIEW,
THE COUNTY OF HAT.F. AND STATE OF TEXA~ AND AUTHORIZING THE MAYOR
TO EXSLTTi~E SAID CONTRACT ON BEHALF OF THE CITY OF PIA]/hS/IEW.
WHEREAS, the contract between the City of Plainview, the County of
Hale and the State of Te~s for the operation of the Plainview - Hale
County Health Department has expired; and
WHEREAS, there continues to be a need for services rendered by the
Plainview - Hale County Health Department; and
WHEREAS, the Texas Department of Healthhas submitted a Local
Health Department Contract (TDH Document No. C5000552) for approval and
execution bythe City of PlainvieWandtheCounty of Hale; and
WHEREAS, the City Council of the City of Plainviewdesires to
continue the operation of the Plainview -HaleCountyHealthDepartment:
NOW, THEREFORE BE IT RESOLVED by the Mayor and the city Council of
the City of Plainview that the Local Health Depa~-b~-~nt Contract (TDH
Document No. C5000552), a copy of which is attached hereto, be and is
hereby approved and that the Mayor be and is hereby authorized to
execute said contract on behalf of the City of Plainview.
